Allow user to select a Gerrit category and post score under it.

      Add settings allowing user to configure scre settings for Gerrit.
      There should be next settings:

      • Post score? checkbox;
      • No violations (number field, default +1)
      • Violations (number field, default -1)
      • Category ("Code review" by default, but should be possible to use user categories)
