Username and password vanish from UI

      Subversion Authentication page at /hudson/scm/SubversionSCM/enterCredential
      sometimes hides the Username and password fields even when the Username/password
      radio button is selected. It is necessary to choose a different radio button and
      then rechoose Username/password.

      This usually occurs when using a bookmark or a back button to reach that page.
