Could not see Username field on "Add Credentials"

      Couldn't see Username field -
      Steps to reproduce:-
      1) Navigate to
      Jenkins Home page --> Credentials --> Global credentials
      2) Click On Add Credentials
      3) Username field is hidden under the OK Button. (Check attached image)

      workaround:-
      select a different option from the 'Kind' drop-down list for e.g. "Certificate", reselect "Username with password" option from the drop-down to see the Username field.

          This is not a 2.1.x series release of the Credentials as the breadcrumb bar does not include details of the credentials store. Thus I claim it is a duplicate of JENKINS-24662 which was fixed in 2.0.4
